Fellowship Sports Interest Form:
The purpose of this form is to notify the membership
committee of your interest in joining or organizing a team to engage in a
fellowship sport with other church members. If you are interested in more
than one sport, you can select multiple sports from the list below by holding
down the Control key while you click on the sports of interest. Please include
your name and email address below so that the fellowship sports coordinator
may contact you. You can use the additional comments field for any information
you think may be helpful. If you select "Other Sport" from the list,
please explain what the other sport is, such as bowling, tennis, croquet,
etc.
